Insurance Coverage

Most insurance companies, including Medicare, now cover the diagnosis and treatment of symptomatic varicose veins and chronic venous disease. If our Physician finds your therapeutic vein care treatment to be "Medically Necessary” and if the treatment regimen is covered as an available benefit under your insurance company's varicose vein policy criteria, then insurance coverage for your vein treatment should exist. If our Physician finds that your vein care treatment is "Cosmetic" according to your insurance policy, we cannot bill insurance, and you will be responsible for prepayment of any "Cosmetic" services or procedures rendered.

We have Physicians which are participating providers with Blue Cross Blue Shield of North Dakota, Blue Cross Blue Shield of Minnesota, Medica, PreferredOne, Benefit Plan Administrators, Medicare, as well as other private insurers. We may also accept insurance programs at our discretion for plans not listed above. You will need to bring your insurance card with you to your appointment. Please call our office to get more information regarding your specific insurance plan. The CPT codes for the endovenous laser ablation procedure are 36478 (first vein treated by EVLA), and 36479 (second & subsequent veins treated by EVLA). The CPT codes for the sclerotherapy procedures are 36471 (multiple injections), and 76942 (ultra-sound guidance during injections, if applicable).

Most insurance plans require members to pay a deductible, co-payment or co-insurance. Depending on your benefits, you may be responsible for partial or full payment of your vein consultation and/or therapeutic vein care procedures. If you have any questions regarding your insurance coverage you may either contact your insurance company or our office.

For patients having no insurance, or choosing not to bill their insurance, it is expected that payment for services will be made in full prior to services rendered.

In all cases, accounts must be resolved in full within ninety (90) days. Accounts not resolved within ninety (90) days will be referred to an outside collection agency.
